- Immediate eligibility (no waiting periods)
Program Guidelines
- Cannot be combined with another dental membership and/or dental plan
- Nonrefundable
- No refunds or membership fees will be issued if participant chooses not to use dental membership
- Patient must pay their portion of any bill on the same date of service
- Membership is valid for one year after premium payment
- Membership will automatically renew
- A $75 reinstatement fee will apply if membership lapses
- Our membership plan is not insurance and does not qualify as a health plan under the Affordable Care Act
